Permission Slip for a Video Interview:  A permission slip for parents/guardians to sign so that their child could be interviewed.

Context
In my Block Experience at ASU, I did a video documentary on the effects of Mass Media on Children.  We interviewed the children about who they look up to or admire.  In order to video the children, I sent home a permission slip to parents.

Impact
The permission slip was created in order to tell the parents of the children in the class a little about the interview that we were going to videotape.  It insured parents that the documentary would not be broadcasted and that it was only for my learning and development as a teacher.  I will be able to use this type of permission slip when I am teaching in my classroom because these procedures for photographing and videotaping students are always necessary in order to fully inform parents.

Alignment
This artifact is a procedure that ensures compliance with security and child protection (Tech Comp 14.1)
The product also serves as a way to communicate with parents.  It supports student learning because the students were able to participate in an interview/study about heroes.  Sending home a permission slip informs parents about what students are doing in class and helps to foster relationships with them (INTASC #10).  Permission slips also give parents a way to participate in classroom activities.
