http://www.col-ed.org/cur/ Columbia Education Center is a website put together by teachers to use in their own classrooms.  On this site you choose a grade and a subject and it has lots you can choose from.  I really like this website.

http://teachers.net/lessons/ This site categorizes the lesson plans by levels Elementary, Middle, and High School.  It also offers other categories such as games and also gives others a chance to add to the site.
