Permission Slip for Media Activities – This is an example of a permission slip that may be given to parents/guardians in the beginning of the school year.

Context:
This permission slip was created as a suggestion during my Integrating Media and Technology into the Elementary Classroom class at ASU.  It was brought to my attention that because of the growing use of technological tools such as video cameras and audiotapes in classrooms, teachers will need to send permission slips such as this one home for legal reasons, as well as to make people aware of what the upcoming school year has in store.

Impact:
In a school setting, permission slips will be sent home for many reasons throughout the school year.  This form is intended to be sent home with students within the first week of school.  By practicing this act, I am preparing myself as a future teacher to be able to create permission slips much like this one.  By using slips such as this, I will establish a good line of communication with parents from the beginning of the school year.

Alignment:
This permission slip promotes and establishes classroom policies and procedures from the beginning.  By using this form of communication, I am ensuring my students legal and ethical rights (Tech. Comp. 14.1).  Another important use of permission slips is set in motion a good relationship with students and their parents or guardians.  It is important to establish good parent-teacher communication early on in the school year so parents know that they can trust you as a teacher and that they can talk to you whenever something is on their mind (INTASC #10).
