*This is the video permission form that I used during my internship class.  The portion of the letter that is in bold is the portion of the letter that targets the need for parental permission in photographing and video taping students.

Dear Parents:

My name is Mandy Reid.  This Spring semester I will be working with and assisting Ms. McKinney with her fourth/fifth grade students.  I will be coming to Beech Mountain once a week until April.  In April I will take on a full-time internship with Ms. McKinney as my mentor.  I am currently taking the final classes of my bachelor’s degree towards Elementary Education.  This internship is part of my experience at ASU.  I look forward to working with this group of students.  As part of my experience, I will be giving a survey about to the students.  I want to find out how their age level views gender roles and how the media has influenced their thinking.  I will also be observing students throughout the semester and would like your permission to photograph and video some of the students If you do not wish for your child to be a part of those photographs and videos, please indicate that below.  Thank you for your time.
