The hands on this picture represent the hands of Klaus and Hannah.  This section goes into detail about the war and the relief efforts which is represented by the bomb and Red Cross symbol.  Klaus and Hannah start to become like a family in the section and become very close, which is why their hands are touching.  Klaus' hand partially covers the bomb because he hates to talk about the war and people treat him horribly because of the war.
