Permission Slip 

Context

This permission slip was handed out to my third grade classroom at the beginning of my pre-service internship with Bethel School. I needed the permission slip to be able to research for my documentary in my Integrating Media and Technology into Teaching course.

Impact

Due to the fact that I would be doing a documentary, taking several photos of classroom activities and projects, and studying several students, I needed to make parents aware of their rights in these procedures.  Along with the permission slip I would send a letter informing the parents of what our classroom would be learning about the media and invite them to view our final product, the documentary.  This permission slip is a great resource for teachers who plan to incorporate video and photography within their classroom.  Permission slips are mandatory to be able to video students and take data on their responses.  With the permission slip on the web teachers can easily access the slip for their own use.

Alignment

This artifact is extremely important for teachers and parents.  I am aware that teachers can find themselves in lawsuit situations if they do not give parents the choice in whether their child can be videotaped or photographed (Tech Comp. 14.3)  This permission slip gives parents a choice and allows them to have a say in the classroom procedures dealing with child protection and security.  Parents would not only be informed with a permission slip, but they would also be made aware of what the students will be learning and discussing; parents will be invited to view the research founded by watching the classroom documentary on the media (INTASC #10).
