Video Permission form requests parental permission to videotape and photograph students during observations and surveys.  It states the purpose for videotaping and photographing, as well as, the purpose of learning about the age group’s developmental levels while performing the survey.

Context
I created this permission form during my Block experience at ASU using Microsoft Word.  It serves as an example of a permission form I would use as a teacher to communicate with parents about what is happening within the classroom.  It also requests their written permission to allow me to videotape and photograph their child.  I didn’t send this form home with the students at Beech Mountain simply because they had already given permission for the year.  Through the letter I inform the parents about my goals and reasons for videotaping, surveying, and photographing.  I will be videotaping and surveying specifically to learn about the effects of media on students’ concepts and ideas about gender roles in society today.  I request permission for students to participate in the videotaping process. This assignment is linked to a permission form created by Melissa Brooks an intern at Beech Mountain the semester before me.  Her permission form was used to get parental permission for videotaping. 

Impact
This particular form is used to receive written permission from parents to allow their students to participate in a project that will further teacher learning.  Teachers can also use this type of permission form to get the consent from parents for a variety of classroom activities that involve student participation such as field trips, science projects, and Physical Education Days.

Alignment
This permission slip would be sent home with the students.  Therefore, I would take time to explain why this permission slip was important.  In gaining permission to videotape, I am following ethical procedures related to technology.  The discussion of social issues, as well as, legal issues is a possibility when talking about the permission form.  For example, I would raise questions such as, “Would you feel comfortable being videotaped if you hadn’t given your permission?”  “Would it be okay for me to take your answers and not tell you that I am going to?”  “Why do you feel this way?”  “Why would it not be okay?”  The permission slip serves as protection of the students’ rights (Tech Competency 14.1).  I used Melissa Brooks' permission form as a guide to guide me in creating my own form, therefore, I gave credit to her.  I would use this as a model for my students, showing them proper citation and how it works.  This is an example of legal issues concerning technology and the use of others work.  I would continue the discussion by asking question such as, “Is it fair for me to take the work of others and use it in my document without citing where the information came from?”  “Why do you think this?”  “What can we do to make it fair?”  Teachers can enhance students’ knowledge about the basic rules of using technology correctly (Tech Competency 14.3)
