This artifact was the document I used in order to get permission to videotape my Portrait of a Learner student. Before I was allowed to start video taping my student , it was extremely important that I complied with the correct measures for doing a project like this (14.1). I was able to acquire permission with the signature of a parent or guardian on the document, so I could begin video taping my student.
