Video Permission form- which states the purpose of video taping students and requests parent permission to have students photographed or video taped, for learning purposes

Context
This permission form was created using Microsoft Word. I created the letter during my Block intern experience at Beech Mountain Elementary School.   I sent a permission form home with each student, requesting their parents’ permission to have them video taped for my own learning purposes.  The letter explains to parents that I am interning in their children’s classroom for the semester and will be photographing and video taping in the classroom to better understand student learning styles and specifically to research the effects of media on student learning and thinking processes.  The letter also asks permission of parents for their children to be a part of this video taping process.

Impact
This permission form/letter could be used by teachers to request permission of parents for their children to take part in video taping, done to further teacher knowledge on student learning and the effectiveness of their teaching methods.  Permission forms such as this could be used to request parent permission for a variety of classroom activities involving students.

Alignment
This permission form is a way to prevent the misuse of technology.  By obtaining parental permission before video taping students, a teacher is following the ethical procedures related to technology.  He/she is only taping students whose parents agree with the video taping procedure.  In sending permission forms home with students, a teacher should explain to students why it is important to get their parents’ permission before taping them, to ensure that students understand the basic “rules” for correct use of technology (Tech. Competency 14.3).
