Student Release form for Media Materials: to be used by the classroom teacher at the beginning of the school year so that parents can decide whether or not they want their child to be videotaped, audio taped, photographed, and or have their work photocopied. 

Context
This permission slip was developed during my pre-service training at Appalachian State University during the Fall 2000.

Impact
Parents will be asked to give permission to the teacher to use their children in Media Materials.  This permission slip also gives the parents the decision of whether or not their children will be allowed to use the Internet.  This permission slip, once signed, gives the teacher the right to use the child in media.  However, no pictures or videotapes they make will be able to be distributed to the public.  The teacher can show it to their supervisors and fellow teachers.  The teacher will establish this policy as one that will be used at the beginning of every year.

Alignment
Teachers who distribute this permission slips to parents are demonstrating their awareness of the social, legal and ethical issues that are related to technology (Tech. Comp. 14.3)
