Permission slip: This is a permission slip given to parents, asking for permission to videotape or photograph their child. 
 

Context
During my media class, Integrating Media and Technology into the Elementary Classroom, I made a media literacy video, which involved videotaping the children.  Since I needed their permission to put their child on videotape, I sent this permission slip home to the parents.  Parents were asked to fill out this form, sign it, and return it to me. 

Impact:
As a teacher, I will need to send permission slips home for a variety of reasons.  By already using this document in a classroom setting, I will already know how to do this. I could also use this as a standard to go by when I send permission slips home to children in the future.  This 
permission slip or letter also lets me communicate with the parents of my students. I would also discuss the permission slip with children so they will know why I was sending it to their parents. I would explain that children need to be protected and they are by using this permission slip.

Alignment: 
This permission slip procedure ensures compliance with child protection. Permission slip would be something I used for a variety of situations making this a classroom policy (Tech Comp 14.1).

Since I have sent a permission slip home with parents that ensures child protection, this shows that I have communicated with parents to support their child’s well-being (Standard 10).
