This Student Release Form is simply one way in which a teacher can gain permission from parents to use their child’s photograph, quotations, and copies of their work on the Internet, or in a video production.  This artifact shows one example of how a teacher needs to receive permission from parents in order to use their children in publications.  

Context 
I created this release form using Microsoft Word during my student teaching experience at Bethel Elementary School, under the supervision of Dr. Pamela Schram and Mrs. Carla Greene.  I created the form in order to gain permission from my student’s parents to work on the web site I created during student teaching involving my students.

Impact 
An item such as a student release form can be used a source of communication to parents and students about what a teacher is doing to fulfill licensure requirements.  This document can be a safeguard for a teacher.  If a parent protests their child’s name as well as footage being on World Wide Web, and that very parent has signed and dated a form such as this, the teacher will be protected. 

Alignment 
This Student Release Form is a model of ethical practices related to technology use (Goal VIA).  This artifact is evidence that I understand and comply with copyright laws and children protection.  If a parent does not choose to sign the student release form, I will be held accountable if I use that child's face in my product.  There are many legal and ethical issues in the teaching profession, and teachers must get permission for everything from parents, as well as document everything relating to that child.  In order to model these copyright laws and procedures, I plan to explain such forms to be sent home to the students so they are aware of the practices that must take place to ensure the legality of one's actions.
