Student Release Form:

Context:
As a requirement in my Media and Learning class at Appalachian State University, I must produce a video of a certain child in my interning classroom. I will video the child working on school-work, socializing, and playing. A permission form must be signed from the child's parents and according to law, must be given to me in order for me to videotape or photograph the child.

Impact:
This permission slip can be obtained via the internet. I will as a teacher, have the permission slip on file for every student in my class from the beginning of the year. I shall discuss that students have rights on that matter and that our goal is to protect them, the children.

Alignment:
The North Carolina Advanced Technology Competency Addressed is (14.1). This states that teachers have the responsibility to provide protection to our students.
