Artifact
Permission Slip, a form used to gain parent's permission to document their child's learning.

Context
   This permission slip was created during my internship at Beech Mountain Elementary School during my pre-service training from Appalachian State University.

Impact 
   During my internship, I was required to videotape interviews with children, collect samples of children's work, and take photographs of our students. Before these actions were taken, permission from the child's parents was required. Parents are informed that their child's learning is being documented.

Alignment
   This permission slip is an appropriate and beneficial professional practice. Permission slips ensures a child's safety and protection (Tech 14.1). In addition, I would discuss with my students why permission slips are important and how they can protect their learning (Tech 14.3). I will use permission slips similar to this one in my future classroom to ensure that parents are informed of the happenings in their child’s education.
