  • Permission Slip

  • During my block experience at Appalachian State University, I participated in a project called “Portrait of a Learner” in which I followed one student carefully learning about her as a student and a learner.  We were required to collect samples of the child’s work in the class, and photograph and videotape him/her.  Since permission for doing this is required by law, I obtained a permission slip and had the parents of the children involved sign it.  In my classroom, I plan to have a permission slip similar to this on file for each student at the beginning of the year.  I will also discuss with my students their rights involving this matter and why it is important to them.
A policy was developed for the use of videotape and photographing in a classroom project to protect the children involved.  The permission slip ensures that the child is protected and the parents are aware and consent with what is occurring in the classroom (14.1).
