Artifact #12:
Permission Slip: a brief note sent to parents and guardians to notify them of my intentions in videotaping and photographing their children as well as how some of their work may be reviewed.

Context:
This is an example of a permission slip created in Microsoft Word.

Impact:
This would be an important note to send home with all of my students.  It shows my concern with confidentiality as well as an interest in whether or not they approve of using their child’s image in photos and videotapes for professional use.  Some parents may feel uncomfortable with these issues, which is something I would deal with delicately and would work to be sure that their child was not included in anything that they did not approve of.

Alignment:
This permission slip ensures child safety and security as far as media in my classroom is concerned (Tech. Comp. 14.1).  If a child’s image should not be made public, then that child’s parent or guardian must inform me of this.  This permission slip informs the parent or guardian that I intend to use media in the classroom that may capture their child’s picture as part of my professional work as an intern in the class.
