Artifact 3:  Permission Slip Form
The permission slip form gives the right to film/tape the students and their work.

Context:
The permission slip form was created in the Integrating Media and Technology into Teaching course at Appalachian State during my pre-service training in the fall of 2000.

 Impact:
The permission slip form will be used to allow the teacher to use a child’s work for discussion of a specific subject.   In class, the teacher will need to talk with the students about the social, legal, and moral issues that surround technology today.  The permission slip will be used to help set up some of the classroom policies and procedures that are to be followed.

Alignment:
In using the permission slip, I will discuss  the need for the permission slip with the students and talk about why such a policy would be needed in the class (Technology Competency 14.1)  Also, by sending home the permission slip form, I will be communicating with the parents in order to get information on how they feel about their student being filmed or taped which allows me to build a better relationship with them and their child(INTASC Standard 10).
