  • Permission slip for Portrait of a Learner

  • During my block experience at Appalachian State University, I followed one student carefully to learn about her as a learner and a student.  This project was called “Portrait of a Learner”.  We were required to collect samples of the child’s work, photograph and videotape him/her.  In order to complete this project I had to get permission from the student’s parents. 

    The permission slip for my Portrait of a Learner student was copied and pasted off of the Media Literacy Assignment Web Page.  It was given to my Portrait student’s mother to gain permission to use the student’s work and to record her progress.  It was used to inform the parent of my intentions for the project, which were to document my student in video, pictures, and samples of work to understand more about how children learn and develop. 

    The permission slip corresponds to the NC Advanced Technology Competencies for Educators and guidelines for professional practice. It is a procedure that ensures compliance with security and child protection policies (14.1).  I plan to use permission slips similar to this one in my classroom to ensure that parents are informed and are in agreement to having their child’s learning process documented.
