- Permission Slip
During my internship I was required to follow a student throughout the semester. This project involved obtaining footage of the student working and interacting with other students. I was also required to keep examples of her work and conduct assessments in reading, spelling and math. The fist step in this process, however, was to send a permission slip home with the child explaining what I would do for this project and receiving permission from the parents to proceed.
As a teacher, I will need to send permission slips home for a variety of reasons. This document gives me a standard to go by when I will ask for permission to document all of my students’ work. It also allows me to effectively communicate with the parents of my students.
