Permission Slips for Media Activities

Context

This request was composed for my Integrating Media and Technology with Classroom Instruction course during my pre-service training at Appalachian State University.

Impact

This form will be sent home with students at the beginning of the year.  Parents must grant permission for the child to participate in videotaped, audiotaped, or photographed activities used in the classroom.

Alignment

This form establishes classroom policies and ensures parent/guardian compliance with the activity pertaining to security and confidentiality, and child protection (Tech Comp 14.1).  Having this initial contact with parents will be beneficial in creating a positive parent-teacher relationship by keeping the parents involved in the education of their children (INTASC 9).  It also encourages parent-teacher communication regarding classroom activity (INTASC 10).
