Artifact: Student
Release for Media Materials: a permission slip for parents to ensure
child protection
Context: I had to use this form during my Internship at Appalachian State University during the fall of 2000, in a first grade classroom. Impact: This form was sent home with my first graders to get permission from their parents to use their work, video tape them, and record their voices for projects that I had to complete in class. The children’s parents were given the option of allowing their child to be audio taped, videotaped, or photographed. I will be able to revise this permission slip for Student teaching and for teaching and use it to provide safety of my students. Alignment: This artifact is an example of how I would ensure security
and child protection of my students. I sent this permission slip home for
my students’ parents to be aware of things that I would have to do in class
and to gain permission to have their child participate in anything I needed
help with. When using this permission slip in an upper grade classroom,
I would be able to sit down and talk to them about the importance of the
