I had to obtain permission from the parents of a student in the classroom, so I could perform assessments regarding his learning. This artifact was used during my internship at Appalachian State University to follow the guidelines of child protection.
As a teacher, I would use this artifact in my classroom. I would send this permission slip home with each student in my classroom, so I would be able to document each student's work.
